About the Role

Connect2Hampshire, a managed service agency owned by Hampshire County Council, is seeking a skilled Social Worker to join their Discharge to Assess Team on a 6-month contract.

The ideal candidate will possess experience in completing Care Assessments with a good working knowledge of Continuing Healthcare and be able to work collaboratively with partner agencies to promote independence and improve well-being for older adults and those living in the Hampshire community.

Key Responsibilities
  • Carry out face-to-face assessments countywide, either in-person at clients' homes or in nursing/care facilities
  • Attend monthly meetings in Basingstoke
  • Refer to relevant agencies and conduct joint assessments as necessary to ensure all risks and needs are considered
  • Monitor, review, and reassess care packages/services against identified risks, needs, and objectives to guarantee continued effectiveness in achieving required outcomes
Requirements
  • A minimum of 3 years' experience in social work, preferably in Continuing Healthcare
