About the Role

As a Network Navigator at North London NHS Foundation Trust, you will play a vital role in ensuring that each patient's network of care is involved early in admission to inform care planning and timely discharge.

Main Responsibilities
  • Work directly with patients to establish details of their network of care, including carers, community mental health teams, and housing providers.
  • Communicate with key stakeholders to establish early care planning meetings and ensure effective discharge planning.
  • Collaborate with multidisciplinary teams to assess risk and develop individualized care plans.
Requirements
  • Degree in a relevant field (e.g., nursing, social work) or equivalent experience.
  • Experience working with vulnerable populations, particularly those with mental health issues.
  • Strong communication and interpersonal skills, with ability to build rapport with diverse individuals.

Estimated Salary: £30,000 - £40,000 per annum
