About the RoleWe are seeking a highly motivated and experienced Programme Delivery Coach to join our team in North West London. As a Programme Delivery Coach, you will be responsible for delivering the Counterweight Plus programme to individuals, providing evidence-based nutrition and health behaviour change support to assist individuals to make changes to their eating and activity behaviours.
Key Responsibilities:- Deliver the Counterweight Plus programme to individuals either in person or remotely using the Counterweight App or Workbook.
- Provide evidence-based nutrition and health behaviour change support to assist individuals to make changes to their eating and activity behaviours.
- Facilitate all sessions using motivational interviewing skills to guide and support individuals to make long term behaviour changes.
- Offer a personalised, person-centred service to individuals and adapt the programme delivery to meet their individual needs.
- Maintain accurate and up-to-date clinical records using our secure electronic medical record system.
- Engage in ongoing training relevant to your role and evidencing professional development.
- Work with your Counterweight colleagues to contribute to service developments.
- Essential: Associate or Registered Nutritionist (AfN) / Registered Dietitian (HCPC) / UKHCA Registered Health Coach / Trainee Health or Clinical Psychologist (HCPC/BPS approved programme).
- Essential: Willingness to undertake the Cambridge Diabetes Education Programme or SCOPE obesity training as part of your role.
- Desirable: Previous experience in delivering a Type 2 Diabetes Remission programme, or working with people living with diabetes, overweight or obesity.
- Desirable: Fluency in Punjabi, Gujarati, Tamil, Polish, and Arabic.
