About the Role

We are seeking a skilled and compassionate Community Nurse to join our innovative Hospice team. As a key member of our multidisciplinary team, you will provide high-quality care to patients with life-limiting conditions and their families in both home and outpatient settings.

Key Responsibilities
  • Deliver holistic care to patients, assessing their physical, psychological, spiritual, and informational needs.
  • Work closely with the multidisciplinary team to provide advice, support, and symptom control to patients and their families.
  • Participate in a structured competency programme to develop your skills and progress to Band 6 within 6-12 months.
  • Establish effective relationships with multiprofessional community teams, including GPs.
  • Act as a patient advocate, liaising with other colleagues and signposting as necessary.
  • Ensure continuity of care through comprehensive recording and thorough verbal handovers.
Person Specification
  • Registered General Nurse (Level 1)
  • Car owner and driver
  • Evidence of Continuing Professional Development
  • Palliative care experience is desirable
  • Community nursing experience desirable
  • Proven experience of working effectively in a team.
  • Ability to perform assessment, planning, implementation, and evaluation of nursing care.
  • Good team player who is willing to support others/learn new skills.
  • Flexible working to defined shift patterns including weekend working.
