Job Summary

We are seeking a highly motivated and compassionate Support Worker to join our team, working nights to provide exceptional care and support to our service users.

Key Responsibilities

  • Provide person-centered support that promotes the health, wellbeing, and independence of our service users.
  • Develop and implement tailored support plans, working collaboratively with our service users, their families, and our multidisciplinary team.
  • Deliver high-quality care and support, including personal care, tenancy support, health needs, personal development, and emotional support.
  • Contribute to the development and implementation of our service users' individual plans, ensuring their needs are met and their goals are achieved.
  • Participate in ongoing training and development opportunities to enhance your skills and knowledge, ensuring you remain up-to-date with best practices in care and support.

Requirements
