Main Duties
  • Support the implementation of medication optimisation initiatives, including medication reviews and medicines reconciliation.
  • Work closely with our Clinical Pharmacists to ensure effective and efficient use of medicines.
  • Develop and maintain relationships with other healthcare professionals to support the delivery of high-quality patient care.
  • Contribute to the development and implementation of practice policies and procedures related to medication management.
  • Participate in quality improvement initiatives to enhance patient care and safety.

Person Specification
  • Professional registration with the General Pharmaceutical Council (GPhC).
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Relevant advanced theoretical and practical knowledge of Primary Care Networks, General Practice, and evidence-based medicine.
  • An appreciation of the NHS agenda and Government targets.
  • Awareness of systems to support management of patients in a primary care setting, delivering pharmaceutical input and support in the context of pathways of care and the business of the organisation(s).
