About the Opportunity

CV-Library is seeking a skilled and empathetic Transition Mentor to join our team. In this role, you will work closely with young people who have left care, providing guidance and support to help them navigate the challenges of transitioning into adulthood.

The estimated salary for this position is £29,500 per year, based on the location and industry standards.

Key Responsibilities
  • To provide guidance and support to young people who have left care, helping them to navigate the challenges of transitioning into adulthood.
  • To work closely with social workers, managers, and other professionals to ensure the effective coordination and provision of services to young people.
  • To identify and address the needs of young people, including their accommodation, education, employment, and health requirements.
  • To develop and implement individualized plans to support young people in achieving their goals.
  • To maintain accurate records and reports, monitoring the progress of young people and identifying areas for improvement.
Requirements

To be successful in this role, you will need:

  • A degree in Social Work or a related field.
  • Experience working with vulnerable young people.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
