Job Overview
We are seeking a highly motivated and experienced Mental Health Support Worker to join our team in Wisbech. The successful candidate will work closely with our Mental Health practitioners to deliver high-quality care to older people in the community.
The key objectives of our team are to focus on supporting patients and their carers living with functional and organic mental illness in the community. As a support worker, you will work collaboratively with our team to implement care plans, manage risk, prevent hospital admissions, and work with carers to prevent carer burnout and care placement breakdown.
The successful candidate will have experience of supporting people living with mental illness and be able to bring a creative and innovative approach to their work. We are a supportive team and offer opportunities for practice development.
Main Duties of the Job
The post holder will work within our Wisbech Neighbourhood Teams supporting people who live with severe and enduring mental health and dementia. The team cares for people who live in the Fenland and Wisbech areas. The post holder will be required to work in a variety of locations such as nursing / residential homes and the patient's own home.
The post holder will be required to take an active role in implementing care and support plans for people as outlined by the care coordinator in collaboration with both paid and unpaid carers.
The post holder will manage delegated tasks for a number of service users, whilst under the supervision of a Band / Mental health nurse/practitioner or above.
