Job DescriptionThe successful candidate will work as a Duty and Assessment Social Worker, responsible for conducting assessments and developing intervention plans to improve outcomes for service users.
Key Responsibilities- Conduct comprehensive assessments, S.47 inquiries, and intervention planning to identify and address social care needs.
- Use paper and computerised case recording systems to document and track progress.
- Develop and implement individual care plans to protect vulnerable individuals and promote their well-being.
- Manage a caseload, providing support and monitoring care packages to ensure high-quality services.
To succeed in this role, you will need:
- Hcpc Registration and 1-2 years post qualifying experience in social care.
- Enhanced DBS/CRB clearance.
