We are seeking a highly skilled and experienced Clinical Team Lead to join our team in St. Albans, Hertfordshire. The successful candidate will have the opportunity to develop their management skills and make a significant impact on the delivery of community health services.

About the Role

The Clinical Team Lead will work closely with the Service Line Lead and management teams to ensure the effective coordination and operational delivery of community health services. This will involve:

  • Assisting the Service Line Lead in delivering a service that meets the Trust's transformation agenda and care pathways.
  • Ensuring the effective delivery of community-based care services through a whole systems approach.
  • SUPPORTING the management of relationships with primary care general practitioners, HCS, PCT, and 3rd Sector organisations.

Responsibilities

  1. Responsible for the provision of quality care within a complex range of health and social care services.
  2. Accountable for the effective operational management of all care pathways, requiring effective liaison and coordination with secondary care clinicians, managers, local politicians, and primary care and other independent contractors.
  3. Manage within established frameworks and processes by which the Strategic Business Unit will manage and monitor capacity on a day-to-day basis, ensuring effective escalation arrangements are in place to maintain safety.
  4. Ensure compliance with statutory requirements, e.g., CQC, HSE, and implement relevant assurance systems to demonstrate compliance.
