Job SummaryThe successful candidate will be responsible for providing expert clinical pharmacy services, including medication reviews, medicines optimisation, and patient education. You will work closely with our Practice teams, Care Homes, and patient groups to improve health outcomes and reduce healthcare costs.
Main Responsibilities- Conduct medication reviews and medicines optimisation for patients with long-term conditions
- Provide education and training to Practice teams, Care Home teams, and patient groups on therapeutics and medicines optimisation
- Reconcile medicines following discharge from hospitals, intermediate care, and admissions to Care Homes
- Identify cohorts of patients at high risk of harm from medicines using clinical databases
- Undertake clinical audits and analyse, interpret, and present medicines data
- Summarise and communicate key prescribing updates and medicines alerts
- Support implementation of local and national guidelines and formulary recommendations
- Monitor Practice prescribing against local dashboards
- Provide newsletters or bulletins on important prescribing messages
- Support PCN Network Practices to fully achieve medication-linked targets and requirements set out in PCN Directed Enhanced Service contracts
