Main Responsibilities

  • Provide personal care and support to patients, promoting their dignity and independence.
  • Assist patients with their rehabilitation and recovery, working closely with healthcare professionals to achieve set goals.
  • Facilitate and lead patient-focused groups and activities, promoting social interaction and engagement.
  • Use approved conflict resolution and management techniques to de-escalate challenging situations.
  • Accurately assess and record patient observations, escalating concerns as necessary.
  • Undertake clinical and environmental risk assessments, documenting key risk areas and seeking guidance from senior staff when required.
  • Provide information on improving health and wellbeing, signposting patients to relevant agencies as needed.
  • Escort patients to other hospitals or care settings as required.
  • Ensure timely and accurate recording of patient information on patient systems.
