About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our Care Homes Service team. As a key member of our team, you will provide high-quality, patient-centered care to housebound patients in care homes across Norfolk and Waveney.

Key Responsibilities
  • Provide proactive health care support to housebound patients, including those in care homes.
  • Work collaboratively with local GP practices and other healthcare professionals to deliver consistent, enhanced services.
  • Contribute to the reduction of avoidable hospital admissions and improve patient outcomes.
  • Support the development of our Care Homes Service, working closely with the Service and Clinical Lead.
Requirements

To be successful in this role, you will need to be registered with the NMC/HCPC and meet the requirements for Advanced Practice. You will also need to undergo a Disclosure and Barring Service (DBS) check.
