Key Responsibilities
- Care Planning: Collaborate with residents, families, and healthcare professionals to design and implement personalized care plans.
- Clinical Excellence: Ensure accurate care documentation, observe daily interactions, and manage specific health needs.
- Leadership: Organize and guide the care team, ensuring clear communication and continuity of care.
- Facilitating Transitions: Oversee shift handovers, ensuring seamless transitions and clear communication.
- Medication Management: Safely administer medications, adhering to MAR sheets and protocols.
- Patient Relationships: Foster strong relationships with residents, families, and healthcare professionals, addressing concerns with empathy and urgency.
- Independence Support: Encourage autonomy while providing hands-on assistance as needed.
- Continuous Improvement: Respond to team input, prioritizing resident health and well-being.
- Respect and Dignity: Uphold the dignity and individuality of every resident, advocating for their needs and preferences.
- Engagement and Socialization: Facilitate social interactions and activities, enriching the lives of residents and creating a vibrant community.
- Safety and Hygiene: Maintain a vigilant eye on health and safety, ensuring a secure environment for all.
- Nutrition and Hydration: Advocate for balanced nutrition and hydration, monitoring residents at risk of weight changes or dehydration.
Requirements
- Active NMC Nursing PIN.
- Passion for care and dedication to improving the lives of our residents.
- Natural ability to connect with people, bringing warmth, understanding, and a calm, patient demeanor to every interaction.
