About the OpportunityThis is a unique chance to work in a collaborative environment, partnering with healthcare providers to deliver community-based care services. As a Community Healthcare Professional, you will play a vital role in ensuring patients receive high-quality care, reducing the need for hospital admissions.
Your primary responsibilities will include working with community diagnostic centres to develop and implement care pathways, coordinating with healthcare professionals to ensure seamless care transitions, and participating in daily morning huddles and 3pm board meetings to review progress and plan for the next day.

- Ensure timely and effective communication with patients, families, and healthcare teams
We offer a competitive salary of £55,000 - £70,000 per annum, depending on experience. You will also enjoy a generous benefits package, including a BMA model contract and NHS pension. Additionally, you will receive 6 weeks' annual leave, allowing you to recharge and pursue your interests.
