Job Overview

We are seeking enthusiastic individuals to join our friendly primary care Mental Health Facilitator team as healthcare support workers. Our team provides Annual Mental Health and Physical Health reviews to individuals with Severe Mental Illnesses across Leicester, Leicestershire, and Rutland.

Key Responsibilities
  • Support clients/service users in maintaining their health and wellbeing.
  • Taking physical (including blood tests) or psychological measurements from named clients/service users as delegated by the team.
  • Supporting clients/service users in maintaining their health and wellbeing.
  • Undertaking specified and delegated clinical and therapeutic activities.
  • Assisting Registered Professionals in the implementation and evaluation of individualised care plans.
  • Accurately recording patient information.
  • Provide support and supervision to other colleagues and learners as appropriate.
