Job Title: Recovery Coordinator

About the Role

As a Recovery Coordinator, you will have the opportunity to work with clients in a prison environment, providing one-to-one support, care planning, risk assessment, and multi-agency working. You will also be involved in delivering group interventions and liaising with partners to support each client's overall health and wellbeing.

Key Responsibilities
  • Provide holistic and structured recovery care coordination and psychosocial interventions to clients, their families, and carers.
  • Collaborate with colleagues, community organisations, and stakeholders across a range of settings to help individuals maximise their sustainable recovery capital and community opportunities.
  • Deliver evidence-based, comprehensive psychosocial programs alongside a range of activities that support the 5 Ways to Wellbeing.
  • Work effectively within the full integrated service model, with a focus on successful and sustainable exit from treatment.
