About the Role

We are seeking a dedicated and compassionate Healthcare Assistant to join our Hospice at Home Team. As a key member of our team, you will provide high-quality care and support to patients and their families in their own homes, helping them to live their final days with dignity and comfort.

Key Responsibilities
  • Work alongside Registered Nurses to provide holistic care and support to patients and their families
  • Assist with practical care tasks, such as personal care, medication management, and symptom control
  • Provide emotional and psychological support to patients and their families, promoting a sense of calm and well-being
  • Collaborate with other healthcare professionals to ensure seamless care and support
  • Maintain accurate records and documentation, adhering to Trinity Hospice's policies and procedures
Requirements
  • Three years' experience in a healthcare setting, preferably in palliative care or community care
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • Basic IT skills, including proficiency in Microsoft Office and ability to use email and calendars
  • Ability to work flexibly, including evenings, weekends, and bank holidays
  • UK driving license and access to a car
