Healthcare Assistant

We are seeking a skilled and compassionate Healthcare Assistant to join our Rapid Response & Intermediate Care Team in Buckingham and surrounding villages. As a member of our team, you will provide nursing, social, and rehabilitative care to patients in their own homes, working closely with senior staff to deliver high-quality care and support.
Key Responsibilities- Provide general and specific care to adults within competencies, including nursing, social, and therapy-based interventions.
- Work as an integral member of the Rapid Response & Intermediate Care Team, providing defined delivery of care to patients in liaison with senior staff.
- Take responsibility for designated elements of patient treatment protocols and delivery of such treatments in patients' own homes.
- Complete and delegate non-clinical tasks within the team, ensuring seamless care delivery.
- Improve the care and support of patients in the community, enabling them to remain safely in their own homes for as long as possible.
- Develop innovative approaches, excellent communication skills, and a willingness to work with other community teams to ensure high-quality service delivery.
- Further develop your clinical expertise in a range of areas, staying up-to-date with best practices and guidelines.

- Previous caring experience in a Health or Social Care environment.
- Awareness of the needs of older people and those with long-term conditions.
- Valid and current driving license and car to travel to locations throughout Buckinghamshire.
- Good interpersonal, verbal, and written communication skills, with the ability to listen and adapt to changing situations.
- IT skills and ability to use technology to support care delivery.
