Job Summary:

We are seeking a compassionate and dedicated Rehabilitation Support Worker to join our team at Chrysalis Brain Injury Case Management. As a key member of our team, you will provide daytime support for a young man with a brain injury based in Hull.

Key Responsibilities:

  1. Assist the client in attending rehabilitation therapy sessions.
  2. Provide support with activities of daily living, including medication management and meal preparation.
  3. Foster a supportive and encouraging environment to enhance the client's well-being.
  4. Ensure the safety and comfort of the client at all times.
  5. Collaborate with healthcare professionals and family members to implement a comprehensive care plan.
  6. Work under the instruction of the case manager and a multi-disciplinary team of therapists.
  7. Complete daily shift notes using OneNote.
  8. Provide emotional support and guidance.

Requirements:

  1. Compassionate and empathetic nature.
  2. Previous experience in supporting individuals with brain injuries or similar conditions is advantageous.
  3. Ability to work overtime.
  4. Must possess a valid driver's license and own a reliable car for transportation.
  5. Excellent communication and interpersonal skills.
  6. NVQ Level 2 or equivalent qualification in Health and Social Care is desirable.
