Living Well Coordinator

We are seeking a highly skilled and compassionate Living Well Coordinator to join our multidisciplinary team at Sobell House and Katharine House Hospice. As a key member of our team, you will play a vital role in coordinating and developing our Living Well services, providing holistic support to patients, their families, and carers.
Key Responsibilities- Coordinate practical and emotional wellbeing support to patients, their families, and carers.
- Facilitate sessions and courses, both within the hospice and in the community, as well as online and remote.
- Design and schedule sessions and courses, and further develop the Living Well services in collaboration with the Living Well Lead and wider MDT.
- Accept referrals to agreed criteria and work in partnership with other professionals to deliver co-ordinated programmes of care effectively.
- Assess health and psychosocial needs of individual patients, and provide care to meet these needs through shared decision making.
- Nursing or Occupational Therapy qualification/degree or equivalent relevant professional qualification.
- Registration with professional body - NMC.
- Experience of working as a Band 6 nurse and undertaking complex holistic assessments.
Sobell and Katharine House are NHS hospices within the Oxford University Hospitals Trust, providing care and support for adults with progressive life-limiting illness, along with their families. Our work is enhanced through the support of the Sobell House Hospice Charity and the Katharine House Hospice Charity.
