Job Overview

We are seeking a highly motivated and enthusiastic Living Well Coordinator to join our multidisciplinary team at Sobell House and Katharine House Hospice. This is an exciting opportunity to work alongside our team to coordinate, develop and provide a wide range of wellbeing focused support to our caseload.

Main Duties
  • Arrange, coordinate and develop the Living Well programme of sessions, activities and courses.
  • Provide online/remote support to patients.
  • Work collaboratively with colleagues and patients to promote self-care.
  • Assess patients' health and psychosocial needs to plan appropriate care through shared decision making.
Requirements

We are looking for an individual with excellent interpersonal and communication skills, a team player committed to collaborative working. The successful candidate will demonstrate innovative and confident teaching skills and an ability to assess patients' health and psychosocial needs to plan appropriate care through shared decision making.

Sobell and Katharine House are NHS hospices within the Oxford University Hospitals Trust, providing care and support for adults with progressive life-limiting illness, along with their families.
