About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our Primary Care Network. As a key member of our team, you will be responsible for delivering high-quality patient care and contributing to the development of our network.

Key Responsibilities

  • Clinically assess and treat patients, utilizing expert knowledge of medicines for specific disease areas.
  • Provide specialist expertise in the use of medicines and medicine information.
  • Offer pharmaceutical medication review and health education advice to patients, through face-to-face, telephone, or notes-based reviews.
  • Provide leadership on person-centered medicines optimization and quality improvement.
  • Support practice dispensary operations.
  • Act as a key contact for all medication-related queries.
  • Run clinics focusing on medicines as primary interventions, including anticoagulant monitoring.
  • Conduct care planning, including mental health, admission avoidance schemes, and end-of-life care for patients.
  • Undertake minor ailments triage and direct patients based on clinical need.
  • Capture relevant patient health information during consultations, contributing to quality outcomes frameworks.
  • Offer clinical advice and expertise on medication.
  • Manage the repeat prescribing reauthorization process, reviewing requests for repeat prescriptions and medications nearing review dates.
  • Review prescribing against local and national targets, engaging with the wider prescribing team to address any issues.
  • Perform structured medicines optimization reviews in care homes.
  • Contribute to reducing medicine wastage within practices across the network.
