Clinical Healthcare Specialist

Job Summary
We are seeking a highly skilled Clinical Pharmacist to join our Primary Care Network. As a Clinical Healthcare Specialist, you will be responsible for providing expert advice on medication management and optimisation to patients and healthcare professionals.
Main Duties
- Clinically assess and treat patients, using expert knowledge of medicines for specific disease areas.
- Provide specialist expertise in the use of medicines and medicine information.
- Deliver pharmaceutical medication review and health education advice to patients, via face-to-face, telephone or notes-based review.
- Lead on person-centred medicines optimisation and quality improvement.
- Support practice dispensary.
- Serve as a key contact for all medication-related queries.
- Run clinics where medicines are the main interventions, including anticoagulant monitoring clinics.
- Conduct care-planning, including mental health, admission avoidance schemes, and end-of-life patients.
- Undertake minor ailments triage and direct patients based on clinical need.
- Capture relevant patient health information at all consultations, contributing to the quality outcomes framework.
- Provide clinical advice and expertise on medication.
- Manage the repeat prescribing reauthorisation process, reviewing requests for repeat prescriptions and medicines reaching review dates.
- Review prescribing against local and national targets, engaging with the wider prescribing team to rectify any issues.
- Perform structured medicines-optimisation reviews in care homes.
- Contribute to reducing medicine wastage within practices across the PCN.

Our Primary Care Network supports four local practices and covers a diverse patient population of around 45,000 patients. We are a forward-thinking PCN committed to tackling health inequalities and working closely with other stakeholders and community teams to achieve this.
