Job Requirements

We are seeking a highly skilled and motivated Mental Health Practitioner to join our team at the Essex Partnership University NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to individuals with complex mental health needs.\

About the Role

This is a challenging yet rewarding position that requires strong assessment, planning, and intervention skills. You will work closely with service users, their families, and other healthcare professionals to develop personalized care plans that promote empowerment, independence, and recovery. The successful candidate will possess excellent communication and interpersonal skills, as well as the ability to work autonomously and as part of a multidisciplinary team.\

Key Responsibilities

• Assess, plan, deliver, and evaluate care for service users referred to the Intensive Outreach Team
• Manage a caseload of patients as the named key worker/care coordinator/lead clinician
• Work alongside service users in meeting practical needs, such as activities of daily living, budgeting skills, social structures, leisure activities, etc.
• Develop therapeutic relationships with service users to provide support and assistance
• Participate in mental health promotion/education and provide advice to clients and carers on maintaining good health and preventing relapse
• Administer and monitor the effects of prescribed medication and liaise with medical staff regarding necessary adjustments

Requirements

To be successful in this role, you will need to hold current professional registration with an appropriate body in healthcare or social work and possess extensive experience in care coordinating people in a community mental health setting. A desire to work in a dynamic and supportive environment is essential, as is the ability to communicate effectively with service users, families, and colleagues.\

Estimated Salary

The estimated salary for this position is £52,000 - £62,000 per annum, depending on experience.\
