We are seeking a skilled and compassionate Clinical Care Specialist to join our team at the Ridge Primary Care Network. As an Advanced Health Care Assistant, you will play a vital role in supporting patients in their homes, delivering high-quality care and improving health outcomes.

The ideal candidate will have a relevant level 3 qualification (NVQ/QCF Diploma or certificate of care/Advanced apprenticeship) or equivalent, as well as experience working in a community nursing environment. You will possess excellent IT skills and be proficient in using Microsoft Office and various IT packages.

A full driving licence valid in the UK is also essential. We offer a competitive salary of £30,000 - £40,000 per annum, depending on experience, plus benefits and opportunities for professional growth and development.
