We are seeking a skilled Domiciliary Care Specialist to join our Transformation Unit team.

**Salary:** £24,071 - £25,674 per annum

About the Role

This challenging and rewarding role involves providing end-of-life care to patients in their own homes as part of our Hospice at Home service.

Main Responsibilities
  1. Work independently with a caseload under the supervision of a senior nurse, performing tasks according to specialist nursing need/assessment at end-of-life.
  2. Provide high-quality nursing care as directed by individualised care plans in the patient's own environment.
  3. Offer hands-on practical care and emotional support to patients and their families/carers.
  4. Maintain patient dignity and privacy while ensuring a safe working environment for all.
  5. Recognise and respect the Equality and Diversity of patients and colleagues.
Requirements
  • Excellent communication and IT skills
  • Able to work as part of a team and on own initiative
  • Understanding of basic end of life symptoms and physiology
  • Confidence in using Electronic Patient Records (EPR)
Preferred Qualifications and Experience
  • Current UK driving license
  • Level 3 Care Certificate
  • Good standard of Numeracy and Literacy
