About the Role
We are seeking a highly skilled and experienced Registered Mental Health Nurse to join our Community Mental Health Team in the Black Country. As a key member of our team, you will be responsible for assessing and formulating individual interventions, plans, and outcome measures that promote recovery for adults with mental health difficulties.
Key Responsibilities
- Assess and formulate individual interventions, plans, and outcome measures that promote recovery for adults with mental health difficulties.
- Work collaboratively with other healthcare professionals to provide comprehensive care and support to service users.
- Develop and implement individualized care plans that address the unique needs and goals of each service user.
- Provide education and support to service users and their families to promote recovery and social inclusion.
- Contribute to the development and implementation of policies and procedures that promote best practice in community mental health care.
