Job Title: Community Mental Health Nurse

Job Overview

We are seeking a Registered Mental Health Nurse with adult and community mental health experience to join our team. The successful candidate will be responsible for assessing and formulating individual interventions, plans, and outcome measures that promote recovery, in line with the recovery model and relevant legislation.

Main Duties
  • Assess and formulate individual interventions, plans, and outcome measures that promote recovery
  • Work collaboratively with partner agencies and professional disciplines to provide comprehensive care
  • Develop and implement individualized care plans that address the unique needs of each service user
  • Provide education and support to service users, families, and carers to promote recovery and social inclusion
  • Participate in multidisciplinary team meetings to discuss service user care and develop strategies for improvement
Person Specification
  • Registered Mental Health Nurse with adult and community mental health experience
  • Excellent written and verbal communication skills
  • Ability to work collaboratively with partner agencies and professional disciplines
  • Experience of using a range of therapeutic interventions in a mental health context
  • Knowledge of government directives and current policies
  • Working knowledge of relevant legislation, including MHA 1983
