Experienced Healthcare Assistant with Phlebotomy Expertise

About the Role:
We are seeking a highly motivated and innovative individual to join our Primary Care Network Team as an Experienced Healthcare Assistant with phlebotomy experience. The successful candidate will support practices in the Network to achieve QOF, LES, and DES targets for patients who are housebound and on the complex integrated care (IC) list.
Main Responsibilities:
- Support practices in the Network to achieve QOF, LES, and DES targets for patients who are housebound and on the complex integrated care (IC) list.
- Visit housebound and care home patients for routine monitoring who are vulnerable, have co-morbidities, and complex health needs.
- Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.
- Provide feedback to the monthly IC multidisciplinary team meetings.
- Promote healthy living and encourage uptake of preventative health promotion activities in the Network.
Requirements:
- Care certificate - Healthcare qualification (level 3 or 4) or working towards gaining equivalent level.
- Experience of working in a primary care environment/healthcare setting.
- Phlebotomy certification, training, and experience.
- Immunisation training, certification, and experience.
- Diabetes foot check training and experience.
