Job Title: Experienced Healthcare Assistant with Phlebotomy Expertise

About the Role:

Merchant Street Practice is seeking a highly motivated and experienced Healthcare Assistant to join our Primary Care Network team. As a key member of our team, you will play a vital role in supporting the delivery of high-quality patient care and contributing to the achievement of our network's targets.

Key Responsibilities:

  • Visit housebound and care home patients to provide routine monitoring and support, focusing on patients with complex health needs and co-morbidities.
  • Undertake care plan A, including phlebotomy, blood pressure monitoring, diabetes foot checks, urinalysis, and spirometry as required.
  • Input data into clinical templates and escalate patient needs to the multidisciplinary team as necessary.
  • Provide feedback to monthly IC multidisciplinary team meetings and promote healthy living and preventative health promotion activities within the network.
  • Work collaboratively with the care coordinator to plan visits and provide holistic support to patients.

Person Specification:

  • Essential:
    • Care certificate and healthcare qualification (level 3 or 4) or working towards gaining equivalent level.
    • Experience of working in a primary care environment or healthcare setting.
    • Phlebotomy certification, training, and experience.
    • Immunisation training, certification, and experience.
    • Diabetes foot check training and experience.
  • Desirable:
    • Experience of working in Tower Hamlets.
    • Experience of carrying out home visits.
    • Knowledge of wider social determinants.
    • Competent in the use of MS Office and Outlook.
    • Ability to work flexibly.
