About the Role

We are seeking a highly skilled and dedicated Children and Young People Safeguarding Nurse to join our dynamic team at Wye Valley NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in promoting the health and wellbeing of children in care in Herefordshire.

Main Responsibilities
  • Conduct holistic health assessments for children and young people in care, considering their physical, emotional, and social needs.
  • Prioritize new and unmet health needs, making appropriate referrals and liaising with professionals to ensure timely healthcare.
  • Support and empower children and young people to understand their health and medical diagnoses, encouraging their contribution to individualized health plans.
  • Develop clear, child-centered health action plans based on evidence.
  • Provide age-appropriate health promotion, advice, and education to children and young people in care.
  • Communicate effectively with children and young people, addressing potential barriers to understanding and communication.
  • Actively promote good health and wellbeing, signposting to specialist services as needed.
