Patient Support Coordinator

We are seeking an experienced Patient Support Coordinator to join our team in delivering high-quality care services to older people in the community.
In this role, you will be responsible for providing first-class client support, working closely with our clients and their families to understand their needs and preferences. You will also be expected to maintain accurate records and communicate effectively with colleagues and other stakeholders.
To succeed in this position, you will need to have:
- Excellent communication and interpersonal skills
- A high level of trustworthiness and dependability
- The ability to work in a fast-paced office environment
