Adult Neighbourhood Social Worker

We are seeking an experienced and enthusiastic Social Worker to join our Adult Neighbourhood Social Work Teams in Stockport.
As a key member of our team, you will work with adults with complex needs, developing your knowledge and practice in a supportive and collaborative environment.
Our teams are closely aligned with health partners and are co-located with District Nursing colleagues and third sector partners, facilitating Multi-Disciplinary Team working.
You will play a vital role in supporting the people of Stockport, ensuring person-centred outcomes are met in ways that utilise available resources effectively.
Key Responsibilities- Undertake all duties in line with the Care Act 2014, promoting and supporting wellbeing in your practice.
- Work with adults from 18 years, experiencing a wide variety of challenges in their lives.
- Collaborate with partners across health, social care, and the third sector to support the people of Stockport.
- Ensure person-centred outcomes are met, utilising available resources effectively.
