Job Summary

We are seeking two skilled Occupational Therapists to join our Reablement Team within Adults Social Care. As an OT, you will work closely with Social Workers and Social Care Assessors to promote independence and wellbeing by conducting comprehensive assessments, developing support plans, and utilizing specialist equipment, adaptations, and risk management programs.

  • Collaborate with local individuals to understand their care and support requirements and identify available options.
  • Implement holistic occupational therapy practice, including manual handling assessments and adherence to relevant legislation.
  • Play a pivotal role in connecting people with necessary support services and community resources.
  • Develop your career through training and development opportunities in various pathways.
  • Conduct enhanced DBS checks and adults' barred list screening.
  • Prior experience working in social care and/or health community settings.
  • Demonstrate effective communication and collaboration with clients, families, professionals, and agencies to achieve optimal outcomes.
  • Showcase working knowledge of health and social care policy, housing, and legislation affecting general OT and moving and handling practices.
  • Able to assess and manage complex clinical risks.
  • Self-motivated individual passionate about making a difference in adult social care.
