Job Title: Adult Clinical Nurse Advisor - Complex Care Specialist

We are seeking an experienced and passionate Adult Clinical Nurse Advisor to join our team at Indianitcompanies. As a key member of our community care team, you will be responsible for providing high-quality care and support to adult clients and their families.

Key Responsibilities:
  • Design and Deliver Personalized Care Plans: Develop and implement tailored care plans that meet the unique needs of each client.
  • Lead the Multi-Disciplinary Team (MDT): Effectively manage and coordinate the MDT to ensure seamless care delivery.
  • Provide Ongoing Support and Assessment: Monitor and assess clients' physical and physiological needs, providing ongoing support and guidance as needed.
  • Manage Medication and Interventions: Deliver nursing interventions, including medication management, to ensure optimal client outcomes.
  • Build Strong Relationships: Foster and maintain strong relationships with clients, families, and community teams.
Requirements:
  • Active RGN Registration: Hold an active registration with the Nursing and Midwifery Council (NMC).
  • Leadership and Management Experience: Demonstrate effective leadership and management skills, with experience of managing healthcare assistants (HCAs).
  • Up-to-Date Clinical Skills: Possess up-to-date experience of tracheostomy care and ventilator management within the last 2 years.
  • Excellent Communication Skills: Exhibit advanced communication skills, both verbally and in writing, to effectively liaise with clients, families, and the MDT.
