Mental Health Care Coordinator

We are seeking a skilled and dedicated Mental Health Care Coordinator to join our team at Hertfordshire Partnership University NHS Foundation Trust. This is an exciting opportunity to work on a temporary basis as part of our Nursing Bank, providing high-quality care to our service users.
About the RoleThis role requires a qualified nurse with experience in mental health or learning disability services to work on a temporary basis as and when needed, often at short notice, at Band 5 level.
The successful candidate will have strong clinical skills, effective working relationships with other professionals, and the ability to work collaboratively with the Charge Nurse and the Team Leader to ensure that the service is managed effectively and efficiently in all aspects.
Responsibilities- Assess service user needs, devise care plans, and evaluate outcomes that promote service user empowerment through the process.
- Work in close collaboration with other professionals to provide high-quality, individualized care that addresses the needs of service users.
- Contribute towards effective management and utilization of resources with the service area.
To be considered for this role, you must have:
- A relevant academic qualification, such as RNMH/RNLD/RMN.
- Membership of professional bodies, including NMC.
- Completed Preceptorship and can evidence this.
You should also have IT skills, particularly in MS Word, and experience of assessing and managing clinical risk.
SalaryThe estimated salary for this role is around £25-£35 per hour, depending on experience.
