About the Job:
We are seeking an experienced Social Worker to join our Adult Community team as a Care and Support Specialist. In this role, you will work closely with individuals, families, and caregivers to assess needs, develop care plans, and provide ongoing support.

Job Description:
This is a challenging but rewarding role that requires strong communication and interpersonal skills. You will be responsible for conducting comprehensive assessments, developing and implementing care plans, and managing complex social work cases.

Responsibilities:
* Assess the needs of individuals and their families, identifying areas of support required
* Develop and implement personalized care plans in collaboration with service users, their families, and carers
* Manage a caseload of complex social work cases, providing ongoing support and guidance as needed
* Collaborate with other agencies and organizations to ensure seamless delivery of services
* Provide training and supervision to less experienced staff members

Requirements:
* A recognized qualification in social work (e.g., DipSW or CQSW)
* Significant experience in adult social work, preferably in a leadership or management role
* Excellent communication and interpersonal skills
* Ability to work effectively under pressure and manage competing priorities
* Strong analytical and problem-solving skills

Salary and Benefits:
* A competitive salary of £43,454 - £45,536 per annum
* A market supplement payment of 12.5%
