Key Responsibilities:
- Lead a team of colleagues to enhance the quality of life of the people we support.
- Develop and implement person-centered support plans to promote independence and well-being.
- Conduct care needs assessments to identify individual requirements.
- Recruit and induct new colleagues to ensure a skilled and dedicated team.
- Complete supervisions and appraisals to support colleague growth and development.
- Manage medication competencies and audits to ensure safe and effective care.
- Attend multi-disciplinary meetings to collaborate with other professionals.
- Complete reports to inform service development and improvement.
Requirements:
- Previous experience in a care management role with a focus on leadership and team management.
- Level 3 in Health and Social Care or equivalent qualification.
- Experience working with individuals with autism, complex mental health needs, traumatic backgrounds, and challenging behaviors.
