Job Title: Social Care Specialist - Vale of Glamorgan
Job Summary:
We are seeking a highly skilled and experienced Social Care Specialist to join our team in the Vale of Glamorgan. As a Social Care Specialist, you will be responsible for assessing and supporting individuals with long-term care needs, working closely with citizens, carers, and internal and external partners to provide the best possible outcomes.
Key Responsibilities:
- Undertake assessments of citizens in accordance with statutory duties under relevant legislation.
- Plan and commission services to enable adults with care and support needs and carers with support needs to overcome barriers to achieving personal outcomes.
- Work in partnership with citizens with care and support needs, carers with support needs, and adults at risk to provide the best opportunities for people to live fulfilled lives.
- Negotiate and coordinate applications for resources and work in partnership with internal and external partners on behalf of citizens and carers.
- Undertake case management responsibility for a specific caseload and prioritize workload demands appropriately.
- Maintain up-to-date electronic case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
- Prepare for and participate in supervision and Personal Development Review sessions with supervisor/line manager, and identify personal development and training needs to maintain registration with Social Care Wales.
- Undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions.
- Supervise or mentor other staff or students where appropriate and offer input into training programs and Team Practice Issues meetings where appropriate.
- Take part in office duty rotas and other activities associated with a multi-disciplinary team.
- Ensure compliance with the Council's Financial Regulations, Standing Orders, Policies, and Procedures.
- Implement the principles of the Council's Equal Opportunity Policy while carrying out the above duties.
- Adhere to Health and Safety legislation/relevant Council policies and procedures and take reasonable care for the health and safety of yourself and other persons who may be adversely affected by your acts/omissions.
- Implement the principles of the Council's Environment Policy while carrying out the above duties.
- Report without delay any safeguarding concerns to the appropriate safeguarding officer.
Requirements:
- Experience of working with adults is desirable but not essential.
- Ability to work in partnership with citizens, carers, and internal and external partners.
- Strong communication and interpersonal skills.
- Ability to maintain accurate records and provide statistical data as required.
- Ability to prioritize workload demands and manage a caseload.
- Ability to work in a fast-paced environment and adapt to changing circumstances.
