Care Manager Professional

As a Care Manager Professional, you will be responsible for coordinating, commissioning, and delivering individual packages of care for a caseload of service users within our secondary service.
About the Role:- Act as Care Co-ordinator, carrying a caseload and working with evidence-based interventions.
- Assess needs, formulate care plans, and implement care plans in collaboration with the multi-disciplinary team.
- Monitor, review, and evaluate care to ensure the best possible outcomes for service users.
- Liaise with all agencies, members of the multi-disciplinary team, and service users to provide comprehensive care.
- Supervise Community Support Workers and peers as appropriate to implement elements of the care plan.

Key Requirements:- Registered Mental Health Nurse or Occupational Therapist or Social Worker with current registration with the relevant regulatory body.
- Substantial experience of relevant post-registration practice, managing a caseload, and working in a multi-disciplinary team setting.
- Experience of working with people with a range of severe and complex mental health problems.
