About the Role

We are seeking a highly skilled and experienced Clinical Team Lead to join our team at East London NHS Foundation Trust. As a key member of our Discharge Hub, you will play a vital role in ensuring the timely and safe discharge of patients from our hospitals.

Main Responsibilities

  • Provide clinical leadership and direction to a team of clinical staff, ensuring the delivery of high-quality patient care in acute and community settings.
  • Develop and implement effective discharge plans, working closely with multidisciplinary teams to ensure seamless transitions of care.
  • Collaborate with hospital social work teams and other stakeholders to identify and address patient needs, ensuring the provision of necessary equipment, packages of care, and support systems.
  • Contribute to the development and implementation of policies and procedures, ensuring compliance with NHS legislation and guidelines.

Requirements

  • Diploma or degree in Physiotherapy or Occupational Therapy, with evidence of post-qualifying and continuing professional development.
  • Well-developed experience of working in adult community mental health services, with a strong understanding of statutory and voluntary community services and agencies.
  • Strong assessment, clinical reasoning, analysis, and presentation skills, with effective communication skills both orally and in writing.
  • Knowledge of NHS policies, processes, NICE guidelines, and current legislation, with leadership qualities and the ability to work collaboratively as part of a multidisciplinary team.
