About the Role

The Reablement Worker will play a vital role in supporting individuals to achieve maximum independence with personal care and daily living skills, working in a person-centred manner.

As part of the Integration Home First Agenda between Social Care and the NHS, the Reablement Team will provide a short-term intervention to encourage individuals to reable and regain their independence, reducing the need for hospital admissions and supporting individuals to live safely and independently in their own homes.

About You

We are seeking an enthusiastic and flexible individual with excellent communication and teamwork skills to join our Reablement Team.

You will have a strong commitment to delivering person-centred care and a willingness to work in a fast-paced environment, supporting individuals with a range of needs and abilities.

As a Reablement Worker, you will be responsible for conducting assessments, developing and implementing care plans, and providing support and guidance to individuals and their families.

You will work closely with other healthcare professionals, including GPs, nurses, and therapists, to ensure that individuals receive comprehensive and coordinated care.

We are looking for someone who is empathetic, compassionate, and able to work independently and as part of a team.

Full training and induction will be provided, and you will have the opportunity to develop your skills and knowledge in a supportive and dynamic environment.
