About the Role
We are seeking an experienced Associate Nurse Practitioner to join our team in the Intermediate Care Community setting. As a key member of our multi-disciplinary team, you will work closely with nursing and therapy staff to provide high-quality care and support for patients following discharge from hospital or other settings.
Main Responsibilities
The successful candidate will be responsible for supporting the nursing workforce to provide safe and effective care, ensuring evidence-based practice for patients under the indirect supervision and leadership of a registered nurse (RN). You will be proficient and competent in your role, working within your scope of practice as governed by the NMC Code (2018) and NMC Standards of Proficiency (2018).
Key Requirements
To be successful in this role, you will need to have a Level 5 Foundation Degree in Nursing Associate or equivalent, as well as GCSEs Grades 4 and above or A to C in Maths and English or equivalent level qualification. You will also need to have experience of working in teams under appropriate supervision as part of a multi-disciplinary team, and be able to provide and receive complex, sensitive information.
