Nurse Practitioner Intermediate Care Community

About the Role
We are seeking an experienced and skilled Associate Nurse Practitioner to join our team in the Intermediate Care Community setting. As a key member of our multi-disciplinary team, you will work closely with nursing and therapy staff to provide high-quality care and support to patients in their own homes or in community settings.
Key Responsibilities
- Support the nursing workforce to provide safe and effective care, ensuring evidence-based practice under the indirect supervision of a registered nurse.
- Provide holistic and person-centred care and support to patients, working within your scope of practice as governed by the NMC Code and NMC Standards of Proficiency.
- Participate in audits and service improvement initiatives to drive excellence in patient care.
- Supervise and support Healthcare Assistants, Trainee Nursing Associates, and Pre-registration nurses to ensure seamless care delivery.

Requirements
- Level 5 Foundation Degree in Nursing Associate or equivalent.
- GCSEs Grades 4 and above or A to C in Maths and English or equivalent level qualification.
- Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.
- Ability to provide and receive complex, sensitive information.
