As a Support Worker, you will play a vital role in providing 1:1 support to an individual with Paranoid Schizophrenia and Autistic Spectrum Disorder, enabling them to live independently in their own home.

Key Responsibilities:

* Provide high-quality support to the individual, promoting their independence and well-being
* Develop and implement personalized care plans, taking into account the individual's complex needs and preferences
* Work collaboratively with the multidisciplinary team to ensure seamless care and support
* Maintain accurate records and reports, adhering to Gray Healthcare's policies and procedures

Requirements:

* Full UK Driving License
* Social care experience, ideally within mental health and with some experience of supporting individuals with Schizophrenia
* Experience providing encouragement to a person with complex health needs and Learning Disabilities
* Experience supporting individuals with behaviours which may be considered challenging
* Experience implementing detailed care plans and risk assessments
* Current Right to Work in the UK and have been employed in the UK for at least 6 months
* Enhanced DBS check and satisfactory references
