Clinical Pharmacist

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at Norfolk Primary Care. As a Clinical Pharmacist, you will play a vital role in providing expert medicines advice and support to patients and healthcare professionals within our Primary Care Network.
Main Responsibilities- Provide direct, accessible, and timely medicines expertise to patients and healthcare professionals.
- Work to optimise medication issues to improve patient care, safety, and support clinical staff in the management of patients.
- Be an integral part of the General Practice team, working in one or more practices across the Primary Care Network, as well as part of a wider pharmacist network and multidisciplinary team.
- Act as a conduit of patient medicines information into and out of Acute General Hospitals.
To be successful in this role, you will need to have:
- Experience as a pharmacist in a primary care setting.
- Strong communication and interpersonal skills.
- The ability to work effectively in a team environment.
- Knowledge of medicines management and prescribing.
