Job Description

As a Community Health Manager, you will be responsible for providing expert care coordination services to patients with complex needs in the Merseyside region. You will work closely with our multidisciplinary team to deliver comprehensive care that meets the individual needs of each patient.

Responsibilities
  • Conduct initial assessments to identify patient needs and develop personalized care plans
  • Train and assess support workers to ensure they have the necessary skills to provide high-quality care
  • Collaborate with the MDT to coordinate care and address any challenges or concerns
  • Attend meetings and make referrals as needed

What We Offer
  • A competitive salary up to £38,000 per annum, dependent on experience
  • The opportunity to earn up to £6,000 per year in tax-free bonus based on targets

Requirements

To be successful in this role, you will need to be a qualified RGN with excellent clinical skills in complex care. Your experience in managing ventilators, tracheostomy care, and peg feeds is essential.
