About the Role

We are seeking a dedicated and compassionate Clinical Case Manager to join our team as a Disability Assessor. As a Disability Assessor, you will be responsible for conducting comprehensive assessments with individuals who have a wide range of physical and/or mental health conditions. Your goal will be to gather detailed information about their challenges and how their disability affects their daily life.

Key Responsibilities
  • Conduct thorough assessments with clients to understand the impact of their disability on their daily life
  • Use your clinical expertise to interpret medical evidence and gather findings into a comprehensive report
  • Work collaboratively with clients to gather information and support their decision-making process
  • Develop and maintain effective working relationships with clients, healthcare professionals, and other stakeholders
  • Stay up-to-date with relevant legislation, policies, and procedures to ensure compliance
Requirements
  • Current registration with the NMC or HCPC
  • Minimum 1 year's broad post-registration experience
  • UK experience is essential
  • Proactive, keen to learn, and receptive to feedback
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and manage competing priorities
