Pharmacist Opportunities in Primary Care

We are seeking an enthusiastic and ambitious clinical pharmacist to join our established team at the Leeds Road Practice. Our practice has been supporting the care of patients across South Harrogate and surrounding villages for over 50 years.
As a clinical pharmacist, you will play a key role in providing high-quality patient care and contributing to the provision of medicines management services in general practice. Your expertise in medicines use will help address public health and social care needs, tackling inequality and improving patient outcomes.
This is an exciting opportunity to develop your leadership skills and work closely with other pharmacy professionals across neighbouring practices and the wider healthcare system. You will be part of a multidisciplinary team that includes GPs, ACPs, nurses, HCAs, PAs, and phlebotomists, as well as having access to appropriate clinical supervision.
About the Role- Work as part of a multi-disciplinary team to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
- Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, and those with multiple co-morbidities.
- Be a prescriber or be willing to undertake training to become a prescriber, working with and alongside the general practice team.
- Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients to help tackle inequality.
- Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework (QoF) and enhanced services.
- Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste, and promote self-care.
- Have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare, and help manage general practice workload.
- Develop relationships and work closely with other pharmacy professionals across neighbouring practices and the wider health and social care system.
- Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists, and anticoagulation.
- To provide support to pharmacist colleagues in the clinical aspects of medicine reviews, clinical audit, practice research, and governance.
- Share outcomes of prescribing, clinical, service, and patient outcomes audits within our practice and across the locality in order to drive service and practice improvement.
- To respond to and act on MHRA drug alerts, implementing and communicating any medication changes required as a result.
- To monitor medicines and prescribing expenditure at practice level and propose efficiencies to ensure delivery within budget.
The Leeds Road Practice is a training practice supporting registrars, medical students, and trainee pharmacists. We have a dispensing service to our patients living in our rural locality and support three local care homes through our PCN-based care home team.
SalaryWe offer a competitive salary package, including a basic salary range of £60,000 - £80,000 per annum, depending on experience, plus benefits such as a comprehensive pension scheme and annual leave entitlement.
Requirements- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Masters in Pharmacy degree.
- Successful Disclosure Barring Service (DBS) check.
